Rules for Checking out Translation Equipment

A city employee must check out all equipment. The employee checking out the equipment must provide the following: Copy of ID A completed check-out form The employee must reserve the equipment at least a week in advance to the date it is needed. The equipment can be picked up as early as 24 hours before the meeting. Equipment must be returned within 24 hours after the meeting. In the event that the equipment gets damaged, the Agency or Department for which the employee represents will be responsible for reimbursing the City Manager's Office for the cost of the equipment. The employee must provide the correct Fund/SF, Organization, and Account number from their department/agency. The contact person for checking out the equipment will be Monique Tsang.
