For a Hazardous Materials file review please fill out the file review request form and fax to (510) 238-6739.

Please provide zip codes to the addresses listed. Please identify the types of files you are interested in reviewing. We have HMBP (Business Plan), UST (Underground Storage Tank) and LOP (Soil and Ground Water Remediation) types of files.

Upon receipt of your request a records search will be performed and then you will be contacted. If necessary an appointment will be set up for you to come and see the files. Please give a minimum of 72 hrs for the records search.
